Eycon Policies
To help us provide attentive, timely care and a great experience, we ask all patients to review our office policies. These guidelines are designed to protect reserved appointment time, clarify expectations, and help us serve every patient well.
Appointment Policies
Late Arrivals
Eycon provides a 10-minute grace period for scheduled appointments. Patients arriving after that time may not be seen at their originally scheduled time.​ When possible, we will make reasonable efforts to accommodate late-arriving patients based on schedule availability. If same-day accommodation is not possible, the appointment may be moved to the next available opening or rescheduled for another date.​​​
Cancellation
& Rescheduling
Repeated cancellations or rescheduling can affect our ability to accommodate other patients in a timely manner. Appointments may be canceled or rescheduled up to three (3) times.

After three (3) cancellations or reschedules, additional advance appointment scheduling may no longer be available. Future visits may then be accommodated on a same-day or walk-in basis, subject to availability.

We kindly ask that patients contact our office as soon as possible if they need to make a change to a scheduled appointment.
No-Shows
A no-show is defined as a missed appointment without notice. Patients who miss a scheduled appointment without notice may be charged a $50 no-show fee.
After two (2) no-shows, the patient may no longer be eligible for advance scheduling. Future visits may then be accommodated on a same-day or walk-in basis, subject to availability.

By scheduling an appointment with Eycon, patients acknowledge and agree to these appointment policies.
Treatment Policies
Reservation Fees
Certain services, such as dry eye and aesthetic treatments, may require extended appointment times and advance scheduling. To reserve time appropriately and maintain availability for all patients, Eycon may require a reservation fee for select treatment appointments:

This fee may be required when booking certain treatment appointments.

This fee is non-refundable.

This fee will be credited toward the total balance of the treatment.​
Treatment Scheduling
Treatment appointments are reserved specifically for each patient and often require extended appointment time and advance preparation. We kindly ask that any schedule changes be communicated as early as possible.

A reservation fee may be required for select treatments and is non-refundable. If a patient cancels, misses the scheduled appointment, or chooses not to move forward with treatment as originally scheduled, the reservation fee may still be applied toward a future treatment appointment, provided the patient reschedules and completes treatment within six (6) months of the original appointment date. After that time, the reservation fee will be forfeited.

At this time, treatment appointments remain subject to Eycon’s general Appointment Policies regarding Cancellation & Rescheduling, No-shows, and Late Arrivals.

Insurance and Payment Policies
​Insurance Information
Patients are asked to provide current insurance information prior to or at the time of check-in. This helps our team review eligibility and prepare the visit appropriately.
As a courtesy, Eycon may verify insurance benefits before your appointment. However, verification of benefits is not a guarantee of coverage or payment. Insurance plans vary, and final claim determination is made by your insurance carrier.
Medical & Vision Coverage
Vision plans and medical insurance often apply to different types of services. Some visits, testing, materials, or treatments may be billed differently depending on the reason for the visit, the clinical findings, and the patient’s plan benefits. Our team will help explain this whenever possible, but patients remain responsible for charges not covered by their plan.
Coverage Estimates
Our team is happy to help review insurance information and explain estimated out-of-pocket costs before services are performed. Estimates are based on the information available at the time and are intended to help patients plan ahead. Please note that estimates are not guarantees. Final patient responsibility may change after the insurance carrier processes the claim and determines the amount covered under the patient’s specific plan.

For questions about specific benefits, exclusions, frequency limits, or reimbursement policies, patients are encouraged to contact their insurance carrier directly, as final coverage decisions are determined by the insurer.
Patient Responsibility
Patients are responsible for all amounts not paid by insurance, including but not limited to:

Copayments

Deductibles
-
Coinsurance
-
Non-covered services

Services applied to plan exclusions or benefit limits

Any remaining balance after insurance claim processing
​​Known patient responsibility is due at the time of service.
Self-Pay Services
Services not covered by insurance, or services elected by the patient outside of covered benefits, are the patient’s responsibility.
HSA and FSA
​For eligible expenses, patients may use HSA or FSA funds in accordance with their plan rules. Patients are encouraged to confirm eligibility requirements directly with their plan administrator if needed.

Optical Policies
We are committed to helping each patient select eyewear that fits both visual needs and personal style. Because many optical products are custom ordered and made specifically for the individual patient, we ask that all patients review the following optical policies.
Optical Orders
Optical orders may include, but are not limited to, frames, prescription lenses, sunglasses, lens options or coatings, contact lenses, and other optical products ordered through Eycon. Because optical orders are often custom made, full payment is required when the order is placed, and all optical purchases are final and non-refundable.
Returns & Exchanges
In select cases, Eycon may allow a frame exchange within 30 days of purchase. Any approved exchange applies to the frame only and does not guarantee that the original prescription lenses can be reused in the new frame. If the new frame is not compatible with the original lenses, or if new lenses are otherwise required, the patient will be responsible for the cost of replacement prescription lenses and any additional optical charges.

Contact lens orders, including unopened boxes and custom contact lenses, are final and non-refundable.

If there is a clear issue related to the prescription, product, or order, Eycon will review the concern and determine the appropriate resolution, which may include an adjustment, repair, remake, or frame exchange when appropriate.
Prescription Changes & Adaptation
Patients who have concerns about their prescription or difficulty adapting to new lenses should contact Eycon as soon as possible. We ask that any prescription concerns be brought to our attention within 60 days of pickup so our team can evaluate the issue and determine the most appropriate resolution. If a change is needed, Eycon will review the concern and may recommend an adjustment, prescription check, or lens remake when appropriate.
Warranties
Manufacturer warranties, if available, are subject to the terms and limitations of the manufacturer. Eycon does not guarantee replacement for damage caused by misuse, accidental breakage, loss, or normal wear and tear.
Insurance & Coverage Information

Please note that vision benefits vary by plan, and coverage for frames, lenses, contact lenses, lens options, coatings, and other materials may differ from carrier to carrier. Patients are responsible for any costs not covered by their plan.

Privacy Practices
Protecting patient privacy is an important part of the care we provide. Eycon handles personal health information in accordance with applicable privacy laws and maintains a Notice of Privacy Practices that explains how medical information may be used and disclosed, as well as patient rights regarding health records.
